	Application Process

	All interested participants must complete and submit an online application and questionnaire which seeks detailed responses regarding the applicant’s business, information technology capabilities and needs. Prior to entering an application, entrants must meet eligibility requirements and accept the general terms and conditions of entry and participation. Please press review rules to browse or download the General Rules and Terms. In order to be eligible for the program, completed entries must be received by 11:59pm Pacific time, June 30th, 2008. Prospective candidates for an Extreme IT Makeover will be notified by July 15th, 2008. The online application process typically requires about 20 minutes from start to finish. To assist you in preparation of your application, press review application form to browse or download the questionnaire, or press apply now if you are ready to proceed. Once your application is submitted, you will receive a confirmation email at the address you provide.
